Stomach cancer is one of the most common cancers in Hong Kong

 

According to CHP,  gastric cancer ranked sixth among the most common cancers in Hong Kong in 2019. There were 1,303 new cases of gastric cancer (Among them, 759 in men and 544 in women). The initial symptoms of gastric cancer are not obvious. Many patients just treat it as gastric discomfort and take it lightly, delaying diagnosis and treatment. Therefore, when more than half of the patients realize the disease and receive treatment, it usually has spread to other tissues, and the disease has reached the middle and late stage.

Preparations

Within 6 weeks before the test Stop taking antibiotics

Within 1 week before the inspection

  • Stop taking anticoagulants such as aspirin or anticoagulants such as Warfarin.
  • Other medicines such as high blood pressure, diabetes, heart medicine or those who are sensitive to any medicine, need to consult the doctor before taking it

Check day

Eating is prohibited 6 hours before the examination, and eating and drinking are completely prohibited for the first two hours.

 

Precautions before inspection:

 

  • Avoid wearing tight clothing before the test
  • Wait two hours after the test or be fully awake before eating
  • Dentures, jewelry and metal objects need to be removed before the examination.
  • Always tell your doctor if you suspect pregnancy.
  • Tell your doctor about any medicines you are taking, especially medicines for diabetes or medicines that affect blood clotting.

Progress

To ensure a smooth gastroscopy, your doctor will prescribe a local anesthetic sprayed in the throat and sedative medication intravenously. Patients should follow the doctor's instructions to breathe through the nose and relax. During the gastroscopy examination, a long and thin tube with a lens on the front end will pass through the patient's mouth, pass through the esophagus, and enter the stomach and duodenum, so that the doctor can observe the health of the internal tissues. If necessary, The doctor can work on polypectomy for patient.

Complete check

The entire gastroscopy usually takes about 10 minutes. After the examination, you need to stay in the center for 1 to 2 hours. After the nurse observes the situation and wakes up, you can leave. The nurse will return the DVD, photos and endoscopy report to the client.

Notes after inspection:

  • Wait 1 hour after the examination or be fully awake before eating.
  • After the inspection, do not drive the vehicle or operate the machinery. It is best for patients to go home with their family members.

Colonoscopy can prevent colorectal cancer at early stage

Colorectal cancer is one of the most common cancer killers in Hong Kong. According to the data from the Hong Kong Cancer Registry and Statistics Center in 2019, there were 5,556 new cases of colorectal cancer in Hong Kong, and 2,174 deaths, making it the second leading cancer killer in Hong Kong. According to statistics, most cases of colorectal cancer are caused by intestinal polyps. Early detection and removal of polyps can help reduce the incidence rate. Polyps can be divided into three types: benign, non-cancerous and malignant. Among them, malignant polyps may have genetic mutations, develop into colorectal cancer, and even spread to other parts of the body.

 

Even if the colorectal polyp is diagnosed as benign, over time, there may be a chance for gene mutation to become a malignant tumor. There are no symptoms during the whole process, and it can be latent for as long as 7 to 10 years. Early detection and removal of polyps can effectively prevent the formation of colorectal cancer or increase the survival rate.

 

Colonoscopy is widely used as an effective diagnostic method before diagnosis, and can accurately find out the cause and potential risks before obvious symptoms appear.

 

High-risk group of colorectal cancer

  • Over 50 years old
  • Previous history of intestinal polyps
  • Family members have had bowel cancer
  • Prolonged diarrhea
  • Bloody stool
  • Positive fecal occult blood test
  • Change in bowel habits (eg, constipation, diarrhea)

 

In addition to the above high-risk factors, people who has experienced sudden changes in bowel habits, bloody stools, sudden weight loss for unknown reasons, and frequent abdominal discomfort should also be careful.

 

Colonoscopy preparation

3 days before inspection
  • Drug Advice Requirements: Please inform the doctor in advance if the patient is taking aspirin and thin blood pills.
  • Stop eating high-fiber foods and drinks: such as vegetables, fruits, wheat bran, wheat bread and fruit juices with fiber
  • Can eat: meat (fish, chicken, duck, goose ) ; eggs (stewed eggs, boiled eggs, fried eggs, steamed eggs or scrambled eggs); milk (whole fat, low fat, skim milk); soybeans products ( soy milk, tofu,)

The day before the inspection

  • Only eat liquid food, such as porridge, milk, etc. (A small amount of shredded chicken and minced fish can be mixed in according to individual needs.)
  • Use KleanPrep, a bowel laxative prescribed by a doctor , mix the laxative with 1 liter of water and take 3 times as directed. Each dose must be consumed within 1 hour
  • After taking laxatives, you will have a bowel movement without pain or discomfort. Stop taking immediately if severe dehydration occurs.
  • If severe pain or extreme discomfort occurs during defecation, stop taking it immediately and seek medical treatment.

Check day

  • Eating is prohibited 8 hours before the examination, and eating and drinking are completely prohibited for the first 2 hours.
  • Arrive 30 minutes earlier and bring your friends and relatives alongside with you

During the cystoscopic examination, the patient lies supine, injects lubricant with anesthesia into the urethra, the bladder will feel full, and then the flexible cystoscope is inserted into the bladder through the urethra. Tissue inspections will be taken if necessary.

The entire colonoscopy examination will take around 15-30 minutes to complete. After the examination, you need to stay in the center for one to two hours. After the nurse observes the situation and wakes up, you can leave. The nurse will return the DVD , photos and endoscopy report to the client.

Notes after inspection:

  • After the examination, if the patient has been injected with sedatives, the patient can have food only after the sedative has worn off to prevent choking.
  • After the examination, their consciousness and thinking ability may be affected. Therefore, it is not suitable for the patient to operate heavy machinery or drive on that day to prevent accidents. Patients should also not sign any legal documents.
  • If patients experience slight discomfort, or have doubts about examination results or medication, they can consult the attending doctor.
  • In case of symptoms such as gastrointestinal bleeding, severe abdominal pain, fever, etc., you should go to the nearby emergency room for medical treatment.

Cystoscopy can help diagnose causes such as blood in the urine, incontinence, overactive bladder, and painful urination. It can also diagnose bladder problems such as bladder cancer, bladder stones, and cystitis. Cystoscopy is to use a very soft and thin tube with a lens on the front to enter the bladder through the urethra in order to observe the patient's lower urinary system, including the urethra, prostate (male) and bladder for abnormalities, such as stones, Ulcers, defects, bleeding or tumors, etc., with mild sedative drugs and local anesthesia, most patients will not feel any pain. If a tumor  is found during the examination, further diagnosis should be follow as the next step.

Notes after inspection:

  • You can eat normally and drink plenty of water to prevent a urinary tract infection
  • There may be a little pain or blood occurring when urinating, which will gradually subside after few days
  • Rest for 1-2 hours after the examination before going home
  •  Drink more than 2 liters of water per day, but not excessive
  • If you have difficulty in urination, abnormal pain, fever or severe blood in the urine, you should seek medical advice immediately
